SUPPLIER DETAILS 

If you have read the trading terms page, are happy to proceed, and we have asked you to provide your details so that we can consider adding you to our list of suppliers, please download, save, and complete the below Supplier Details Form which will open in Microsoft Excel.

Please only complete and return this form if you have already contacted us and we have asked you to proceed to this stage of the process. Please ensure you provide all the requested bank details and that they are accurate.  If you don't, we can't pay you!  We generally pay into UK based GBP bank accounts so if you are not based in the UK, please contact us for advice before proceeding.

SAMPLE PAPERWORK

Please prepare a sample delivery note and a sample invoice that includes sample data as explained on the supplier details form.

SUBMITTING INFORMATION

You must email the below to your hmv head office contact and all in the same email. 

• Supplier Details Form (completed in full and in Excel)

• Sample Delivery Note

• Sample Invoice

• Business Continuity Plans (where appropriate)

• Modern Slavery Policy (where appropriate)

• New account forms for us to complete (if necessary)

• Any other relevant information

We shall reply to confirm that we have received the information and will let you know if we have any queries.

DRAFT AGREEMENT

Once we have reviewed the details you supply, we shall prepare a draft agreement and will email you a copy to review. If you have any questions regarding the draft agreement, please direct them to your hmv head office contact. 

By supplying the requested information, we are under no obligation to open an account with you and there shall be no trading agreement between us until such time as a formal agreement has been signed by both parties.
